Here is a little .doc that I made up for your important information. You can either type in your information using a program that can edit Microsoft Word Documents or you can print the paper out then fill it in. Lots of free programs for this on the net. Google has one and you can download OpenOffice also.

My Mom worked in an Emergency Room for over 20 years, she's an EMT, and was a volunteer Fire Fighter for many years so I trust her judgement when it comes to what information is needed by a First Responder. Remember, you may not be able to give this information to someone even if you are awake when help arrives.

I suggest putting this paper in an ID holder around your neck. This should help the suit protect the information. An EMT is going to be opening your suit to check for injuries so this would place it in a very noticable location.

Please feel free to give me suggestions on other information you suggest be added. You can share this with any board you like, but I just ask that any improvements to the paper make it back to me so I can update the files that I distribute. Consider this an Open Source project. :D

this may be corny, but maybe there could be a thread x'in for riders who have a ride location, times, and ride buddies in mind. maybe after a roll call at the gas station, and text a x'in with a mobile. to the "sunday paly ride". and then x'out. i dunno. i'd do it.

as a first responder, i think its a great idea, it could even be kept with a drivers license. this gives important info when assessing a patient, just as stated above, in a true emergency, the patient might not be able to talk, and those riding with them, might not know the info.

I hope you get a lot of deja vu on this topic. It would mean people are planning ahead. :D

I like the who's on what ride idea. I've been meaning to print out a map of my "planned" ride for the day so my wife would know how to get to me if I, or someone with me, were to go down and I need the truck. I guess you could act like you are going hiking alone in a big park. You always tell someone where you are and where you are heading. Then again, you should never ride or hike alone. :)
